SCHEDULE OF VERBAL ASSESSMENT

(Starting May 4, 2020)

 

Monday to Friday – Grades 1 to 5

Online Assessment Registration

  1. Please prepare the scanned copies of the assessment requirements:
  2. Report Card with Learner Reference Number (LRN) - Recent Quarter
  3. NSO/PSA Birth Certificate
  4. 1”x1” recent I.D. Picture

  1. Click Register for Verbal Assessment
  2. Fill out the required fields. Make sure that you give a correct and active e-mail address (preferably a GMAIL account).
  3. Upload required documents
  4. Click Data Privacy Consent Statement and Register
  5. A verification letter will be sent to your email within 1-2 days  (Please check your email regularly)

Kindly prepare the following before going online:

  •       Laptop with internet connection and headset
  •       (2) Sharpened Pencils with eraser
  •      Bond Paper/any clean sheet of paper
  •       Set of Crayons (For Nursery and Kindergarten ONLY)

(Please be online 15 minutes before the assessment)

 ENROLMENT REQUIREMENTS WILL BE EMAILED TO THE REGISTRAR

  1. Report Card
  2. Photocopy of the following:
    a. Authenticated Birth Certificate (PSA)
    b. Baptismal Certificate for Catholics/Certificate of Dedication for Non–Catholics
  3. Good Moral Certificate
  4. Recommendation Letter
  5. Copy of Clinical Diagnostic Report (if any)
  6. Dental Certificate for Grade 1 only

 

OTHER REQUIREMENTS FOR FOREIGNERS:

  1. Special Study Permit from the Bureau of Immigration
  2. Photocopies of Visas & Passport (parents and student)
  3. Notarial Certificate/Narrative/Red Ribbon
