OPTION A

1. The student downloads the Request Form. Please click here.

2. The student fills in and signs the form.

3. The student gets access to the link: https://forms.gle/1khAJLW7wTVgas4M8, fills in the necessary data and uploads the ff:

a) Scanned copy of the completed online request form.
b) Scanned copy of front and back sides of student’s XU ID or any government-issued ID.
c) Scanned copy of a 2” x 2” photo with white back ground for students of the Juris Doctor program or if the purpose of the request is for board/bar exam

4. The Registrar’s Office conducts initial clearance of the account of the student at the Finance Office and notifies those who still have unsettled financial obligations

5. The student waits for the assessment of payment by the registrar on the document and courier fee. The documentary fee depends on the highest obtained degree while the courier fee depends on the location of the delivery address.

6. The student makes a payment deposit for the assessment given by the registrar with the ff details:

                              Name of Bank:  Bank of the Philippine Islands

                              Account Number: 9330-0016-55

7. The student gets access to the link: https://bit.ly/36PsrJ6 , fills in the necessary data and uploads a copy of proof of payment with the student’s name written on it and the online assessment form.

8. The Finance Office confirms the payment and issues an official receipt.

9. The Registrar’s Office notifies the student on the status of his request. Turn-around time for the TOR is 5-14 working days from date the payment was cleared and posted by the Finance Office while that of the diploma is 1 month.

10. The Registrar’s Office secures clearances from the ff offices: Library, Dean’s Office, Office of Student Affairs, Book Center, Finance Office and Guidance and Counseling Office (additional for transfer credentials) . Requests will not be processed if student has incomplete credentials (F138, F137-A, Official TOR from previous school, Transfer Credential, etc.) The registrar will notify those who still have unsettled financial and/or property obligations with the forementioned offices.

11. The Registrar’s Office sends the requested documents via LBC or DHL to the student’s address indicated in the request form.

Option B (same as Option A except for steps 5 and  11)

5. The student waits for the assessment of payment by the registrar on the document and scanning fee. The documentary fee depends on the highest degree completed. The scanning fee depends on the requested document.

11. The Registrar’s Office sends a scanned copy of the documents via the student’s email address. The hard-copy is kept at the Registrar’s Office for at most six months from the time the campus shutdown has been lifted.

Option C

2. The student fills in and signs the form.

3. The student gets access to the link: https://forms.gle/1khAJLW7wTVgas4M8 , fills in the necessary data and uploads the ff:

a) Scanned copy of the completed online request form.
b) Scanned copy of front and back sides of student’s XU ID or any government-issued ID.
c) Scanned copy of the claim stub of his previous request.

4. The student waits for the assessment of payment by the registrar on the courier fee. The courier fee depends on the location of the delivery address.

5. The student makes a payment deposit for the assessment given by the registrar with the ff details:

                              Name of Bank:  Bank of the Philippine Islands

                              Account Number: 9330-0016-55

6. The student gets access to the link: https://bit.ly/36PsrJ6 , fills in the necessary data and uploads a copy of proof of payment with the student’s name written on it and the online assessment form.

7. The Finance Office confirms the payment and issues an official receipt.

8. The Registrar’s Office notifies the student on the status of his request. Documents will be dispatched for delivery 1-2 days from date the payment was cleared and posted by the Finance Office

Option D (same as Option A except for steps 5 and 11)

5. The student waits for the assessment of payment by the registrar on the documents. The documentary fee depends on the highest degree completed.

11. The Registrar’s Office emails a scanned copy of the claim slip to the student indicating the schedule for the pick-up of the documents. Please read separate guidelines for the pick-up of the documents.
