Dear Student:

Please accomplish our online form so that we can print your University ID.  

Before you proceed, please read the guidelines for the instructions on how to prepare the following requirements:


For New Students (Freshmen who are not from XUSHS, transferees and returnees)

Old Students* (Freshmen who are from XUSHS and those enrolled in AY 2019-2020)
