Online Admissions Application
School of Medicine Master’s Degree Program
First Semester SY 2022-2023

 

I. Fill-out Application for Admission

   a.) For Continuing or Returning Students https://forms.gle/m51cmkdw3GCAwJx8A with the following:

  • Latest Transcript of Records (complete copy, certified true copy-scanned)
  • NSO-authenticated Birth Certificate (scanned)
  • Marriage Contract, if applicable (scanned)
  • School/Company ID or any valid ID (scanned)
  • Essay: Statement of Purpose, 600-800 words

   b.)  For Transferee or New Student: https://forms.gle/ETiKRSvHxC9g4PPz8

  • Transfer Credentials or Honorable Dismissal (XU graduates who enrolled in another school after graduation from XU, are required to submit the transfer credentials from last school attended) (photo copy or scan copy)
  • Please include all requirements at a. For Continuing or Returning Students

A notification will be sent to set a schedule for online interview thru Google meet starting at 9:00 A.M. Panelist are GS Dean, Program Chair & Coordinator. Then an Approved study load and guide for enrollment fee will be emailed to you.

II. Pay down payment. A minimum enrollment down payment Php 4,000.00 is required to pay at the Finance Office or any of the accredited payment centers found in this link. https://www.xu.edu.ph/payment-centers-finance

Upon receipt of your proof of payment, you will be endorsed for term activation. Wait for the term activation notification from Registrar and Computing and Information Services Office(CISO), then you can proceed to self-enrollment at Student Life Cycle and Information System (SLMIS).

III. Proceed to self-enrollment via SLMIS https://slmis.xu.edu.ph/psp/ps/?cmd=login&languageCd=ENG&
