GOOD MORAL CERTIFICATES GUIDELINES

I. REQUESTS:

1. Online GMC Request is accepted through xu.edu.ph/osa.  
2. Options are provided either PRINTED or ELECTRONIC COPY (PDF Format). Printed copy shall bear the signature of the school official and dry seal for authentication purposes while electronic copy shall have a QR Code for verification purposes. 
3. All requests shall have a fee of Php 50.00.
4. For authenticated copies, an additional fee of Php 10.00 per copy is charged. (Not applicable for electronic copy).
5. All payments should be made through the following account (either via bank deposit, fund transfer, GCash/Maya, or other online payment platforms). Payments should be made first before making a request online.

6. Proof of Payment (e.g. scanned copy, screenshot, or photo of the deposit slip) must be uploaded to this form. Requests will not be accepted without a valid proof of payment. 

II. RELEASING

1. Printed copies may be picked at OSA personally or through a representative. For pick-up by a representative, an authorization letter, a copy of any valid ID from the requesting person, and any valid ID of the representative are required.
