Secondary Uniform

 

The student must be taking the prescribed practicum, fieldwork, or lab work subject/s for the current semester.

 

1. Accomplish the on-line form.
2. Proceed to the Book Center (G/F Main Library Building) to place your order. Your order will only be accommodated once OSA indicates an “approved” remarks on your online request.  
3. Pay the required fee.
